Please also bring with you your insurance card with your identification number as well as relevant information about your plan. We will be happy to work with you in determining coverage through your plan and in filing claims for reimbursement.

It is our policy to exhaust all third party payer responsibilities before billing our patients directly. If a patient is unable to make full payment of the remainder of his or her bill, every effort will be made to schedule a payment plan which is fair and agreeable to both parties. In most cases, Medicare assignment is accepted.
